Company K — combining environment and OH&S in one system and sharing common procedures with quality: from a legal-record nonconformity to an integrated management review
Company K, which ran ISO 9001 only, added ISO 14001 and ISO 45001 as one system and shared common procedures such as documented information, risk, internal audit, corrective action and management review with quality. The legal register, aspect and OH&S risk assessments, control plans, objectives, integrated audit and management review are linked by the same numbers. A fictional case for training.
This case is a fictional scenario for training purposes. The company, people and figures are invented examples. Clauses are given only as the common numbers of the harmonized structure (HS, clauses 4 to 10). The Korean edition uses Korean laws; here they are shown as types of legislation. Check the original text of the laws that apply in your country, and do not take any article number, statutory frequency or deadline from this page.
Today’s situation
Company K is a mid-sized machinery-parts maker. Its processes are cutting and pressing, machining (cutting fluid), washing, powder and liquid painting, and assembly and shipping. It has 180 employees (example) and has held ISO 9001 certification only. This year it is adding ISO 14001 and ISO 45001 as one system, and it is using the procedures common to all three standards together with quality. The plan runs for one year (example): build, operate, internal audit, management review.
The people involved are the Environment and Safety Team Leader, the Health and Safety Officer and the Quality Team Leader, with approval by the CEO. The numbers in this article (L, EA, EC, HR, OB, NC, IA, MR) stay the same from start to finish.
The company first created a Company Workspace and added the three people as author, reviewer and approver. The operational-data screens need an operations pass for each standard held by the workspace administrator; without one they are view-only (see eiso Pricing).
Step 1: Designing the integrated document structure (folders)
The company opened Integrated Management System Document Development.
What was entered on the screen
- "Standards to integrate": ISO 9001 · ISO 14001 · ISO 45001
- "Documents to integrate": the existing quality manual and 18 quality procedures (example)
- "Outputs to create": integrated manual, common procedures, document structure table
- "Requests": "Do not create a common procedure per standard; keep one for the company. Use the 00·10·20·30·40·90 numbering and keep the current numbers of quality-specific procedures."
- "Customer materials": the existing manual and procedures, organization chart, process flow chart
Pressing "Write with AI" showed the price; after agreeing and paying, the draft came out.

Tip: Put "one common procedure per company" and the numbering in "Requests" first, so the same procedure does not come out three times. Keep standard-specific procedures to work that only one standard covers (clause 8 operation).
Step 2: Identifying legal requirements and the legal register
In Management System Operation the company chose the Social Value track, ISO 14001 · ISO 45001 as the standards, and Integrated operation as the mode, then opened Legal Requirements Identification. ISO 9001 stays in the Economic Prosperity track and is combined only in the common steps (see Step 4).
What was entered on the screen
- "Industry (size, main products)": metal processing and machinery parts, 180 people (example), brackets and shafts
- "Department or process": cutting and pressing, machining, washing, painting, assembly and shipping
- "Situation and data summary": 2 paint booths, 1 washer, 12 presses, 4 forklifts (example), chemicals list
- "Requests": "Do not write article numbers or statutory frequencies; mark them (check the original text)."
The departments reviewed the result and entered it with L numbers in the Assessment tab (compliance obligations register) of Social Value Operational Data.
Preview of the result
| ID | Legislation (type) | Equipment or work | Gist of requirements | Records | Owner | Review (company) |
|---|---|---|---|---|---|---|
| L-01 | Air quality legislation | Paint booth emission and control equipment | Emission equipment control, self-monitoring and record keeping (check the original text) | Self-monitoring log, contractor test reports | Environment and Safety | Half-yearly |
| L-02 | Waste legislation | Spent cutting fluid, waste paint, packaging waste | Segregation and storage, licensed disposal and transfer records (check the original text) | Waste log, transfer records | Environment and Safety | Half-yearly |
| L-03 | Chemicals legislation | Paint thinner and washing agents | Whether substances are regulated, handling and labelling (check the original text) | Chemicals list, determination record | Environment and Safety, Purchasing | On new use and half-yearly |
| L-04 | Water quality legislation | Washing wastewater | Whether the discharge is regulated, discharge and operating records (check the original text) | Wastewater operating log | Environment and Safety | Half-yearly |
| L-05 | Occupational safety and health legislation | Presses, forklifts, paint booths, solvent work | Risk assessment, inspection, exposure monitoring and health checks where applicable, training (check the original text) | Risk assessment, inspection and training records | Health and Safety Officer | Quarterly |
| L-06 | Serious-accident (corporate duty) legislation | Whole site (top management) | Checking that safety and health duties are carried out (scope and frequency: check the original text) | Duty checks, management review minutes | CEO, Health and Safety Officer | Half-yearly |
Tip: The review column holds only how often the company rereads the register. Statutory measurement or inspection frequencies stay as "(check the original text)" until the owner has read the law.
Step 3: A record nonconformity from compliance evaluation, and corrective action
In July (example) the company ran its first-half compliance evaluation (CE-26-H1) in Compliance Evaluation. L-01 showed two things: one round of self-monitoring records for the paint-line air emission equipment was missing, and the contractor test report was in the production team’s cabinet, not in the place the register named (example).
In Nonconformity and Corrective Action the team entered this in the situation summary and chose "Add operational-data summary" to get a draft cause analysis and corrective action. Pressing "Open corrective action" on the flagged compliance row creates NC-26-07 in the Nonconformity and Corrective Action Register.
How it was handled
- Correction: a copy of the report was obtained from the testing contractor and filed in the named place. Whether the missing round breaches a legal duty was checked against the original text and with the competent authority.
- Why-why analysis: Why was it missed? The schedule was only in one person’s calendar. Why? The register had no fields for planned month, record location and receiver. Why? EP-30-03 covered "doing" the measurement but not "receiving and filing" the report.
- Corrective action: three fields were added to L-01, EP-30-03 was revised from Rev.1 to Rev.2 to include confirming receipt of the report, and IMS-P-10-01 now names the filing place for the report.
- Effectiveness: effective if the reports for the next two rounds are filed on time in the named place.
Preview of the result: the company uses the same single register as quality and splits it with the "Standard" column and the "Standard" filter.
| NC No. | Standard | Clause | Source | Description | Status |
|---|---|---|---|---|---|
| NC-26-05 | ISO 9001 | 7.5 | Self-check | Missing signature on a final inspection record | Closed |
| NC-26-06 | ISO 45001 | 8.1 | Near miss | Damaged walkway marking in the shipping area | Closed |
| NC-26-07 | ISO 14001 | 9.1 | Compliance evaluation CE-26-H1 | L-01: one round of self-monitoring records missing, report filed in the wrong place | Effectiveness check |
Tip: Do not stop at filling in the record. Change the register (L-01) and the procedure (EP-30-03) together, so the next measurement round does not repeat it.
Step 4: Environmental aspects, significant aspects and the environmental control plan
In Risk Assessment the company switched on "Integrated management system: both tracks together (common steps only)" and added ISO 9001. Risks and opportunities came in one register, with separate sheets for aspects and for OH&S risk. The results went into the Assessment tab of Social Value Operational Data with "Import from workspace results".
Criteria set by the company (example): severity (S), frequency (F) and scale (Q) are each rated 1 to 5 and multiplied. A score of 27 or more, or severity 4 or more under abnormal or emergency conditions, is "significant". Aspects with legal obligations are managed through their L number in the compliance obligations register, whatever the score.

| ID | Process | Environmental aspect | Condition | S·F·Q | Score | Legal | Result |
|---|---|---|---|---|---|---|---|
| EA-01 | Machining | Spent cutting fluid (hazardous waste) | Normal | 3·4·3 | 36 | L-02 | Significant |
| EA-02 | Painting | Solvent VOC emissions | Normal | 4·5·3 | 60 | L-01 | Significant |
| EA-03 | Washing | Washing wastewater | Normal | 3·4·3 | 36 | L-04 | Significant |
| EA-04 | Cutting, pressing | Metal dust (indoor collection) | Normal | 2·4·2 | 16 | None | Normal |
| EA-05 | All processes | Electricity use | Normal | 2·5·2 | 20 | None | Normal |
| EA-06 | Painting | Paint or thinner spill | Emergency | 3·1·2 | 6 | L-03 | Normal (SP-40-04 emergency response) |
| EA-07 | Assembly, shipping | Packaging waste | Normal | 1·4·2 | 8 | L-02 | Normal |
The three significant aspects became the environmental control plan in Process Control (called operational control in the Social Value track).
| Control ID | Aspect | Item | Criterion (company) | Monitoring, frequency | Equipment | Action if out | Record |
|---|---|---|---|---|---|---|---|
| EC-01 | EA-01 | Fluid concentration, stored waste volume | 5–8% (example), closed and labelled containers | Weekly concentration, weekly volume | Refractometer MM-07 | Top up or replace, arrange disposal before storage limit | Fluid log, waste log |
| EC-02 | EA-02 | Paint and thinner use, control-equipment pressure drop | Monthly use within target, pressure drop within company range (example) | Monthly use, daily pressure drop, self-monitoring (statutory frequency: check the original text) | Differential gauge MM-12 | Stop painting, change filters, corrective action | Control equipment log, self-monitoring log |
| EC-03 | EA-03 | Wastewater pH, discharge volume | pH within company range (example; legal limits: check the original text) | Daily pH, monthly volume | pH meter MM-15 | Stop discharge, neutralize, investigate | Wastewater operating log |
Equipment MM-07, MM-12 and MM-15 is managed in Monitoring and Measuring Equipment Control and in the Equipment tab of the Measuring Equipment and Documents Register, where "Enter calibration result" sets the next calibration date.
Tip: Write the criteria as numbers at the top of the register so next year’s reassessment gives the same result. Use the same equipment numbers in the control plan as in the equipment register.
Step 5: OH&S risk assessment, medium-or-higher risks and operational controls
This is the OH&S risk sheet of the same risk assessment.
Criteria set by the company (example): likelihood (L) × severity (S), each 1 to 5. 16–25 is high (act now), 9–15 is medium (improve), 1–8 is low (acceptable). For medium and above, controls follow the order elimination, substitution, engineering, administrative, PPE, and the residual risk is assessed again.
Preview of the result
| ID | Process, task | Hazard | L×S | Level | Controls (in order) | Residual |
|---|---|---|---|---|---|---|
| HR-01 | Press die change | Caught in | 3×5=15 | Medium | Engineering: guard checks / Administrative: lockout and tag, permit to work | 1×5=5 |
| HR-02 | Machining | Flying chips | 3×3=9 | Medium | Engineering: chip covers / PPE: safety glasses | 2×3=6 |
| HR-03 | Paint booth | Solvent vapour fire | 2×5=10 | Medium | Substitution: low-solvent paint trial / Engineering: interlocked ventilation / Administrative: earthing checks, emergency scenario | 1×5=5 |
| HR-04 | Painting | Solvent exposure | 3×4=12 | Medium | Engineering: local exhaust checks / Administrative: monitoring and health checks where applicable (check the original text) / PPE: respirators | 2×3=6 |
| HR-05 | Shipping area | Forklift and pedestrian collision | 4×4=16 | High | Elimination: separate walkways / Engineering: warning lights, rear sensors / Administrative: speed limit, banksman | 2×4=8 |
| HR-06 | Washing | Skin contact with washing agent | 2×2=4 | Low | Keep current controls | 4 |
| HR-07 | Assembly | Manual lifting | 3×2=6 | Low | Keep current controls | 6 |
The controls for HR-01 to HR-05 went into work instructions and operating criteria in Process Control (for example, a lockout step added to WI-31 die change, Rev.3). HR-03 led to scenario ER-02 (paint booth fire and spill, including EA-06) and a drill plan (tabletop in the first half, live drill in the second, example) in Emergency Preparedness and Response. Consultation with worker representatives (whether a statutory safety committee applies: check the original text) was recorded as CM-26-03 in Participation, Consultation and Communication.
Tip: Write the control type next to each control (elimination, substitution, engineering, administrative, PPE) so rows that rely on PPE alone stand out. Put the risk ID in the title of each emergency scenario.
Step 6: Implementation and monitoring
In Objectives and Performance Monitoring the company used "Import from objective setting" in the Objectives tab and linked each indicator to its control ID as the data source. Each month was closed in the Monthly close tab. Measured values come from the Monitoring and measurement tab of Social Value Operational Data.
Preview of the result (April to September, example)
| Objective | Indicator (source) | Target | Apr | May | Jun | Jul | Aug | Sep | Status |
|---|---|---|---|---|---|---|---|---|---|
| OB-E1 | Thinner use, L per 1,000 parts (EC-02) | 10.8 or less | 12.1 | 11.8 | 11.5 | 11.2 | 11.0 | 10.9 | On track |
| OB-E2 | Spent cutting fluid, t per month (EC-01) | 1.7 or less | 2.1 | 2.0 | 1.9 | 1.9 | 1.8 | 1.8 | On track |
| OB-S1 | Near-miss reports per month (all HR) | 20 or more | 8 | 12 | 15 | 21 | 24 | 22 | Achieved |
| OB-S2 | Controls completed, cumulative % (HR-01~05) | 100 | 20 | 40 | 40 | 60 | 80 | 100 | Achieved |
Tip: Adding the control ID to the indicator name lets the management review find the source at once. In a month when a figure stalls, as in June, leave a one-line reason at month close.
Step 7: Integrated internal audit
In October (example) the company opened Internal Audit as a common step with all three standards and got a process-based audit plan and checklist. The result is IA-26-01 in the Internal Audit Register, with three findings entered using "Add finding".
Preview of the result (one checklist, with Q·E·S clause columns side by side)
| No | Process | What to check | Q | E | S | Result |
|---|---|---|---|---|---|---|
| C-01 | Documented information | Document list and review date of external documents (laws) (IMS-P-10-01) | 7.5 | 7.5 | 7.5 | Observation O-02 |
| C-02 | Painting | Action records when EC-02 goes out of range | 8.1 | 8.1 | 8.1 | Conforms |
| C-03 | Shipping | HR-05 controls in place (walkways, banksman) | None | None | 8.1 | Minor nonconformity F-01 |
| C-04 | Compliance evaluation | L-01 record location, NC-26-07 effectiveness | None | 9.1·10.2 | 9.1 | Observation O-01 |
| C-05 | Objectives | Monthly close and actions for OB-E1 to OB-S2 | 6.2·9.1 | 6.2·9.1 | 6.2·9.1 | Conforms |
- F-01 (minor nonconformity): the second shift did not post a banksman; the work instruction had not been issued to that shift. It appeared in the To open tab of the corrective action register and was opened as NC-26-11.
- O-01 (observation): only one of the two effectiveness checks for NC-26-07 is done. Close it after the second.
- O-02 (observation): some rows in the document list have no review date for external documents.
Tip: Do not let auditors audit their own department. Putting the clauses of all three standards beside one question saves asking it three times.
Step 8: Integrated management review
In November (example) the company opened Management Review as a common step with all three standards and held it as one meeting with one set of minutes (MR-26-01), together with the quality review. In the Management Review Register, "Check inputs" showed missing inputs and "Add decision" recorded the decisions.
Preview of the result
| Type | Item | Content (example) | Source ID |
|---|---|---|---|
| Input | Compliance evaluation | One nonconformity among L-01 to L-06, effectiveness being checked | CE-26-H1 · NC-26-07 |
| Input | Significant aspects | Two out-of-range events, both acted on | EA-01~03 · EC-01~03 |
| Input | Medium-or-higher risks | Five controls in place, all residual risks low | HR-01~05 |
| Input | Objectives | Two E objectives on track, two S objectives achieved | OB-E1~OB-S2 |
| Input | Audit and corrective action | One minor nonconformity, two observations, two open actions | IA-26-01 · NC-26-07·11 |
| Input | Participation and consultation | 6 of 9 worker suggestions adopted | CM-26-03 |
| Input | Quality performance | Customer satisfaction, complaints, delivery (existing quality review items) | Quality registers |
| Output | Resources | Automatic pressure-drop recorder for the control equipment | D-01 (EC-02) |
| Output | Objective changes | OB-S1 next year 25 per month, keep OB-E1 as a per-unit indicator | D-02 |
| Output | Improvement | Low-solvent paint trial (shared by EA-02 and HR-03), quarterly register check | D-03 |
Tip: Enter common inputs once and add only the standard-specific ones (compliance, participation, quality performance). Link decisions to the register with "Open corrective action" so they feed the next review’s inputs.
Step 9: Connection map
| Flow | Numbers in this case |
|---|---|
| Legal register → compliance evaluation → nonconformity → corrective action → procedure revision | L-01 → CE-26-H1 → NC-26-07 → EP-30-03 Rev.2 · new L-01 fields |
